Important Update on the Tilbury Chadwell PCN Reconfiguration – 1st April 2025

We are writing to inform you of an important development within the Tilbury Chadwell Primary Care Network (PCN) from the 1st April 2025. To enhance patient care and strengthen our ability to deliver progressive healthcare services, the following practices, Oddfellows Hall Health Centre, Milton Road Surgery and Thurrock Health Centre will be moving under the umbrella of Tilbury Chadwell PCN.

Why This Change?

As a growing and evolving network, our focus remains on delivering high-quality, patient-centred care. By increasing the size and diversity of our network, we will:

  • Enhance our ability to work at scale, enabling the development of new, progressive services for our patients.
  • Strengthen our approach to population health management, allowing us to deliver more proactive and preventative care.
  • Improve health outcomes by ensuring a more integrated, collaborative, and sustainable model of care.
  • Increase opportunities to work with partners to bring out of hospital services closer to home.
  • Expand upon the success of our GP and AHP (Allied Healthcare Professional) PCN Training Hub by attracting more new blood into Thurrock.

What This Means for Patients

Patients will continue to receive care at their existing practice locations with the same familiar staff, ensuring continuity of care. This transition allows us to build on existing services while introducing new initiatives that provide better access, same-day care opportunities, and a wider range of healthcare support tailored to our communities.
